Our social prescribing link workers help patients to get the right support. Many different patients will be referred to social prescribing link workers. They include people with long-term conditions, low level mental health issues and those who feel lonely or isolated. Link workers also help people with complex social needs.
Generally, patients speak with a link worker 6-12 times over a three month period. The link worker will connect them with activities, groups, and services in their community for practical and emotional support to help them get back on their feet.
How a Link Worker can help you:
- Help to overcome barriers that may be holding you back
- Support in creating a plan to boost health, well being, and a support network
- Connections to local services, groups, and activities that suit your needs and interests
- Be there as a guide, cheerleader and problem-solver along the way
Whether it’s housing worries, loneliness, anxiety, or something else entirely – a Link Worker means you don’t have to face it alone.
