Our Enhanced Care Home Team work closely with our local care homes, providing care homes with a central contact for support. They carry out weekly visits to the care homes to provide care to our patients.
The care home team consists of a Lead GP, a Care Co-ordinator, an Advanced Clinical Practitioner and a Nurse. They are also supported by members of our pharmacy team.
